The information in your file consists of data that is – directly and indirectly – important for the support and possible diagnosis and treatment. For instance, reports concerning steps that were made on the way to achieving goals, special incidents, examinations that have taken place and their results, prescribed medication and their effects on your health, findings from discussions with your counsellors etc.
Each one of the people who has access to your file is registered in a so-called authorization document. If necessary for your diagnosis or treatment, Kempenhaeghe may share this information – with your permission – with third parties (care providers, institutions, health insurers or the Dutch Care Authority) Everyone who has access to your information, has a confidentiality obligation.
As a parent, family member or legal representation, you always have the right to access the information on your child or family member. It is also possible to correct certain information or have it removed. For instance, if the information is incorrect, incomplete, not relevant or in breach with Dutch legislation.
In Kempenhaeghe, the data protection official ensures that we work according to the Dutch privacy legislation.
